Root cause

The initiating, most basic cause of a problem that may or may not lead to a chain of causes or other problems. If the root cause is identified and resolved, it should prevent recurrence. The root cause of an event or a problem must be identified to establish an appropriate Corrective and Preventive Action (CAPA) Plan. OHRE: SOP 1401
